Name the aviation context
Identify the FBO or general aviation airport, city, destination, date, timing, passengers, luggage, and vehicle preference. If the facility name is uncertain, provide the airport name or code and explain what is known.
Private aviation requests can differ from commercial airport transfers. The request should avoid assuming access, curb procedures, parking, or a facility-specific meeting point.
Plan the ground segment
Include the business meeting, residence, hotel, event, or other destination and list any stops. If the traveler needs a return movement or a vehicle available between meetings, state that as a planning requirement.
Only provide information needed for the ground trip. Aircraft registration, passport data, and other sensitive aviation details are not required for this form.
Leave facility procedures for review
FBO access and pickup instructions depend on the facility and the actual operating arrangement. The request can establish the itinerary while the business confirms the practical details later.
Submission stores a request and generates a reference; it does not confirm an aircraft-side pickup, vehicle, chauffeur, or price.
