A practical checklist for assistants, travel managers, and meeting planners preparing transportation requests.

Capture the program shape
Record the company or group name, traveler count, dates, pickup and destination locations, meeting or convention venues, and whether transportation is needed for one person, a team, or a schedule of movements.
If the program spans airports, hotels, offices, campuses, or event venues, list those relationships explicitly. A request can then be reviewed as a coordinated itinerary instead of a series of disconnected addresses.
Separate requirements from preferences
Mark the information that is required for the trip from preferences such as a vehicle category, a particular contact method, or a desired stop sequence. Vehicle categories remain requests until availability and suitability are confirmed.
For recurring or multi-day programs, an organized schedule and a point of contact can reduce follow-up. Do not add employee identifiers or other personal information that is not needed.
Plan for review and change
Corporate transportation is often adjusted as meetings and attendee schedules change. Keep the request reference, provide updates through the approved contact path, and treat each change as part of the review process.
A form submission is not a corporate account, dispatch agreement, fixed rate, or confirmed reservation. Those arrangements require a separate approved business workflow.
